Insurance Accepted

SPORT Orthopedics accepts many insurance plans. Below is a partial list of the most common insurance carriers accepted. To ensure your individual plan is in network with our providers, we recommend you confirm coverage by contacting your insurance carrier or your employer’s Human Resources personnel prior to your appointment.

At your visit, please present your health insurance card. We request patients present their insurance cards at their initial visit or if your insurance carrier has changed to verify your benefits and ensure payment from your insurance company. We do not accept Medicaid.

We accept cash, all major credit cards and CareCredit. Payment for services not covered by your health plan are due at the time of your visit
